Conviva Care Centers se une a Strive Health para ofrecer un servicio de gestión de enfermedades renales a los pacientes del sur de Florida

MIAMI Y DENVER – 28 de septiembre de 2020 – Conviva Care Centers, una red de 300 médicos de atención primaria que se especializa en el tratamiento de personas mayores, y la empresa de atención renal especializada Strive Health anunciaron hoy que se asocian para ofrecer servicios especializados de gestión de la atención de enfermedades renales para pacientes elegibles en el sur de Florida.

Strive Health y Conviva Care Centers integrarán la tecnología y la infraestructura clínica para mejorar los resultados sanitarios de los pacientes con enfermedad renal crónica (ERC) y enfermedad renal terminal (ERT). Los equipos de atención de Strive Health trabajarán con los médicos de atención primaria de Conviva, aportando su experiencia en la atención a los pacientes con enfermedad renal crónica y enfermedad renal terminal en fase avanzada.
