La enfermedad renal crónica afecta a más de uno de cada siete adultos estadounidenses, es decir, a unos 37 millones de personas. Cuando los factores agravantes, como la diabetes, la hipertensión arterial, las infecciones o lesiones repetidas y el consumo de drogas y tabaco, no se controlan bien, aumenta significativamente el riesgo de que la enfermedad renal crónica (ERC) avance hasta convertirse en enfermedad renal terminal (ERT). Con cada avance de la ERC, el coste para los planes de salud estadounidenses aumenta exponencialmente. Sólo Medicare gasta unos 130.000 millones de dólares en atención renal, incluidos casi 30.000 millones de dólares para proporcionar diálisis a 554.000 estadounidenses con insuficiencia renal. Además, los pacientes con ERC, incluso en fases tempranas de la enfermedad, soportan una carga desproporcionada de morbilidad, mortalidad, utilización de la asistencia sanitaria y costes cardiovasculares.

Los elevados costes y la alta utilización de la atención sanitaria asociados a la ERC, junto con la prevalencia de la enfermedad, subrayan la necesidad de identificar intervenciones y estrategias que reduzcan los costes y la utilización, prevengan o ralenticen la progresión de la enfermedad y aborden los factores que la agravan para mejorar los resultados de los pacientes.

La identificación de estos impulsores de los costes es importante para la toma de decisiones en materia de política sanitaria, ya que permite una atribución más precisa de la utilización de la atención sanitaria y la intervención sobre otros determinantes de la salud en esta población. Si la ERC se detecta a tiempo, se inicia el tratamiento y se abordan otros factores determinantes de la salud, se puede ralentizar el deterioro de la función renal, retrasar la insuficiencia renal y compensar potencialmente los efectos secundarios de esta enfermedad.
