Introduction and Scope

Strive Health, LLC, and its affiliates (collectively, “Strive,” “we,” “us,” or “our”) are committed to protecting your Personal Information. This Privacy Policy for California Job Applicants, Employees, and Other Professional Relationships (“Policy”) describes how we collect, use, and disclose personal information of California residents (“Consumers”) in accordance with the California Consumer Privacy Act of 2018, as amended by the California Privacy Rights Act (collectively, “CCPA”). “Personal Information” means information that identifies, relates to, describes, references, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular Consumer or household. It is important to us that you understand how we manage your Personal Information. 

This Policy describes how we manage your Personal Information before, during, and after your employment or professional relationship with us. While your professional relationship with us continues, we will update this Policy as required by law. We will continue to protect your Personal Information in accordance with applicable law after your professional relationship with us ends. You do not need to take any action because of this Policy, but you do have certain rights to your Personal Information as described in the California Notice of Collection and Other California Resident Rights section of Strive Health’s Privacy Policy, which is supplemental to this Policy and can be found at https://strivehealth.com/privacy-policy/

Collection of Your Personal Information

In our capacity as an employer, potential employer, or other professional relationship, we may collect several types of Personal Information about you, as applicable and appropriate. It may be information you provide directly to us or to one of our third-party service providers. Service providers are required by law to protect your Personal Information, and the collection, use, and disclosure of Personal Information that you provide directly to our service providers is governed by each service provider’s privacy policy. 

Personal Information may be collected before, during, or after your professional relationship with us. It may be collected by us directly or we may receive it from a company outside of Strive. The Personal Information collected will vary depending on various factors, such as your citizenship, location of residence, roles and responsibilities, or the type of professional relationship you have with us. It may include, but is not limited to, your:

  • Name, contact information, and other demographics;
  • National Provider Identifier (applies to our health care providers only);
  • Employment history and qualifications;
  • Background information;
  • Benefits enrollment information;
  • Facial photographs;
  • Health information (such as for worker’s compensation claims);
  • Financial information (such as bank account details for payroll purposes); 
  • Workplace device, usage, and content information; 
  • Workplace video and/or audio surveillance; and
  • Emergency contact information.

Disclosing Personal Information

We disclose your Personal Information only to those who require it to assist in managing your professional relationship with us. For example, we may disclose your Personal Information to staff within our companies and/or to certain service providers or contractors with whom we partner. We require our staff, service providers, and contractors to protect your Personal Information and to use and disclose it only for authorized purposes or as permitted or required by law. 

We do not sell or share your Personal Information to outside marketers who may want to offer you their own products and services unless we receive your express consent; nor do we disclose Personal Information we receive about you from a consumer reporting agency. You do not need to take any action to exercise this limitation.

Security of Information

We have an important responsibility to keep your Personal Information safe. To protect your Personal Information from unauthorized access and use, we use security measures that comply with applicable law. These measures include device safeguards and secure physical locations. Our staff is authorized to access, use, and disclose your information only when necessary to perform their job responsibilities, and those who have access to your Personal Information must keep it confidential.

Electronic Monitoring

When applicable, we may monitor the electronic usage and communications performed on a Strive-owned device and/or within Strive systems that have been authorized on a personal device, in accordance with applicable law and Strive policies and procedures.

Rights to Your Personal Information

It is important to us that you have access to your Personal Information and that it is both accurate and current. We offer various self-help tools that will allow you to view and update certain Personal Information in our records. 

  • Job applicants may access and update their Personal Information directly within our third-party hiring software.
  • Current employees may access and update their Personal Information directly within our employee management and benefits software(s).
  • Former job applicants or employees, or others with whom we have or had a professional relationship – If we maintain your Personal Information and you are unable to access or update it via a self-service tool,  please complete the California Consumer Privacy Rights Request Form and we will review your request.

Paid Leave
Our 12 weeks of paid birth giving leave and four weeks of paid bonding leave start day one. We provide a four-week paid sabbatical after five years to return recharged. In alignment to our mission, we offer a paid living donor leave if any Strivers choose to become a living donor.
