SSM Health is a Catholic, not-for-profit, comprehensive health system with 23 hospitals and more than 290 physician practices and other care sites in Illinois, Wisconsin, Oklahoma and Missouri. System leaders recognized that early intervention could better serve its kidney disease patients with complex needs — and better manage the associated high costs. 

With the shift to value-based kidney care, SSM Health chose to partner with Strive Health. Together, they have proven their ability to rapidly scale services to a vulnerable and often forgotten population and manage the financial risks of kidney care. 

It took just 90 days for Strive Health and SSM Health to move from an agreement to launch in 2020. First-year results affirmed the partnership’s value: 10% savings in total costs of care among ESKD and high-risk CKD populations and 48% reduction in unnecessary hospitalizations in the ESKD population.

Today SSM Health and Strive Health are building on their partnership by increasing patient participation in value-based kidney care, exploring new relationships with healthcare payors in the Midwest and enhancing the care model to discharge planning, follow-up and multi-modality dialysis.
